Reluctance of faculty to formally report violations: The University of Alberta lists these reasons: perception of the university process as time-consuming & tedious; often the punishments do not fit the crime; reluctance to damage the student's reputation or career; difficulty in proving the accusation; lodging a complaint against a student might reflect badly on the teacher's own teaching skills; it can be a traumatic, emotionally draining experience.

- 1) open form 2) fill out each field, tabbing between fields 3) click on Create Form 4) review your information - if incorrect, use your browser's Back button to return and re-enter information 5) click on Print 6) send form via campus mail to address on bottom of form Note: can you save a copy of the completed form? yes, click on your Browser's Save or Save As, save the file, and then check to see if it has saved successfully.
