Online LPN/LVN to BS Student Checklist

Following is a checklist of procedures you must complete in order to make your experience at Indiana State as successful as possible. If you have questions, contact the office listed or the Office of Admissions toll free at 800-GO-TO-ISU.

STEP 1. ACTIVATE YOUR MyISU PORTAL

Visit isuportal.indstate.edu. Click on “Activate Your Sycamore Login.” You will need your University ID number (found on your letter of admission), your date of birth, and the last four digits of your Social Security Number.

STEP 2.  CONFIRM YOUR INTENT TO ENROLL

Click on the admission confirmation link (below), and let us know you're coming. Then we’ll start preparing for your college experience at Indiana State University.Your transfer credits will not appear on your credit transfer report until you have confirmed your enrollment.

STEP 3. ACCESS AND READ YOUR CREDIT TRANSFER REPORT (MySAM)

After you confirm your intent to enroll, access your report through the MyISU Portal. This report details all of your transfer hours accepted by the University as well as course work you must complete to finish your Indiana State degree. Instructions »

STEP 4. APPLY FOR FINANCIAL AID

Apply for federal and state financial aid by completing the Free Application for Federal Student Aid (FAFSA) at www.fafsa.ed.gov. (The federal school code for Indiana State is 001807.) We have information on all aid programs available online or contact the Office of Student Financial Aid at 800-841-4744

STEP 5. INVESTIGATE VETERANS' BENEFITS

STEP 6. REQUEST ACCOMMODATIONS FOR DISABILITIES (IF APPLICABLE)

CONDITIONS OF ADMISSION TO NURSING

Your acceptance to the nursing program is contingent upon meeting all Phase II requirements within one calendar year of this notification. The requirements are as follows:

  1. Completion of four (4) Nursing Challenge Exams/Customized Study Materials (CSMs). Review the Challenge Exam informationEmail ISU-DistanceNursing@mail.indstate.edu when you are ready to schedule your exam.
       Nursing 106: Mental Health Aspects of Nursing (two credit hours)
       Nursing 224: Nursing Care of Adults I (five credit hours)
       Nursing 328: Nursing Care of the Child and Family (four credit hours)
       Nursing 330: Nursing Care of the Childbearing Family (four credit hours)
     
  2. Test of Essential Academic Skills TEAS® V (Version 5.0) Examination. Contact testing coordinator via email at ISU-DistanceNursing@mail.indstate.edu or by phone at 812-237-2726. The TEAS examination is a 3.5-hour computerized examination that measures abilities in reading, math, science, and English.
     
  3. Completion of the LPN to BS Application to the Department of Baccalaureate Nursing Completion.
       Due June 1, 2014 for fall 2014
       Due October 1, 2014 for spring 2015
       Due May 1, 2015 for fall 2015
       Due October 1, 2015 for spring 2016
