Assessment Plan Template

The assessment plan template serves as a guide for any department developing a new or revising a current program.  A completed form must accompany all proposals for new programs submitted to Academic Affairs. If proposed changes to an existing program will result in the need to revise the assessment plan, the revision should be reflected in the table and submitted to Academic Affairs with the proposal.  To access the assessment plan template, click here.  After the program has received all approvals, the assessment plan must be entered into TaskStream.  For assistance with completing the template, contact the Assessment and Accreditation Coordinator.
