demonstrates an understanding of
1) the differences between
person-centered planning and
other educational planning
strategies, 2) the purpose and
use of different person-centered
planning maps; 3) how to lead a
team through the PCP process and
facilitate a person-centered
planning meeting; 4) how to use
person-centered plan information
to develop goals for IEPs/IFSPs,
transition, employment,
independent living, community
participation and other life
events; and, 5) how to develop
action plans from PCP
information.
Meets Expectation:
Completes three out of five
"Task for Completion and
Discussion" assignments. The
learner can summarize the ideas
involved in understanding the
differences in person-centered
planning and the purpose and use
of different person-centered
maps; how to create the
different maps used in person
centered planning; and
recognizes the values and
benefits of person centered
planning for individuals with
deafblindness and other multiple
disabilities.
