Update Personal Information

Students should review and update their personal information on a regular basis to ensure that the University has accurate information on critical items such as name, social security number, emergency contact , marital status, and mailing address for timely receipt of important information including (if applicable) W-2 Forms for student employees.

Instructions are provided below. For assistance, contact the Office of Registration and Records.

The following information can be reviewed and updated by logging onto the MyISU Portal, clicking on your Portal Profile initials in the upper right corner, and choosing "University Profile" then click on "Personal Information".

  • Emergency contact
  • Phone number

To request a change of name, students must complete and submit this form and supporting documentation (for example, certified copy of marriage license, divorce decree). Information can be submitted in person or via fax to:

To request a change of social security number, students must present their Social Security Card reflecting the correct social security number to:

Note: Students may request a change of social security number by faxing a copy of their social security card and a copy of their driver's license to the Fax number (above). Please include student identification number along with faxed items.
