Letters of reference

Letter of Recommendation Departmental form
Master of Arts requires 3 letters of recommendation
Doctor of Philosophy requires 5 letters of recommendation
Along with our form it is requested that the referee to send a supplemental letter. We prefer the letters to be in a sealed envelope.  We will accept emailed or faxed letters if close to the deadline date, however, we request that a signed original letter follow the copy.

Statement of purpose/intent

This is a brief essay usually no longer than a page describing in precise detail what your goals are not only within the graduate program at ISU but beyond your academic career as well as define why you as an applicant have chosen the graduate program at Indiana State and how our program will help them obtain their goals.

Curriculum vita or Resume

Supplemental Materials

Though supplemental materials are not required for the application process it is highly recommended that the applicant provide additional materials for the review committee.  These documents may consist of: maps, abstracts, conference posters on regular sized paper, and writing samples. 

Official GRE scores

Both the M.A. degree and the PhD degree require GRE scores.  We require that the scores be up-to-date (within the past five years) and scores are to be sent to us directly from ETS.  Our University code is 1322.  Photocopies are not accepted.  To be considered for funding both the verbal and quantitative scores must each be at least 500.  Under rare circumstances do we offer funding to students with one section below 500.   Please do not contact us asking if you would be considered due to lower GRE scores.  The committee reviews all materials in the application and they make that determination and will contact the student.

Transcripts

Transcripts from all attended universities.  University requires a final transcript showing highest degree conferred or awarded.

Official TOEFL scores

The TOEFL needs to be sent directly from ETS, photocopies are not accepted.  Minimum score requirements without exception are:
Paper based 550; Computer based 213; and internet based 80.

Official transcripts and diploma

Submit official academic records (transcripts) and official diploma or certificate demonstrating  successful completion of a four-year bachelor degree program to the School of Graduate Studies.  Academic records for all universities attended must be received in English and the native language. 

Financial Support

Financial support sufficient to cover the duration of the student's first academic year of educational expenses must be demonstrated before admission is granted and the I-20 eligibility document is issued. Submit an official bank statement or an official financial document for proof of financial resources from all sponsors showing the ability to cover expenses for your first academic year at Indiana State University.

Affidavit of Financial Support

All applicants should supply an Affidavit of Financial Support. Government sponsored applicants must provide an official or certified copy of the award, specifying validity for Indiana State University, listing the expenses and fees covered by the award, beginning and ending dates of study covered, the level/field of study, and the length of the award. The address for billing must also be included.
