ISU Health Center

Union Associated Physicians Clinic (UAP Clinic) and Indiana State University entered into a partnership as a cost-effective way to continue providing high quality healthcare to Indiana State students. UAP Clinic leverages its core business as a healthcare provider to assist the University to enhance services available via the Health Center.

UAP Clinic is a primary and specialty care medical practice with nine clinics throughout Terre Haute. The UAP family of clinics, comprised of physicians, nurse practitioners, physician's assistants and more than 500 staff members, offers services in 21 medical specialties.

Indiana State clinical staff at the Health Center have access to state-of-the-art electronic health records.

Our Staff and Services

The clinical employees at the Health Center are UAP Clinic employees. The Health Center is staffed by a nurse practitioner, laboratory and x-ray techs, nurse and support staff. In the fall, physicians supplement the clinical staff at the Health Center.

Services available at the Health Center include immunizations, men's and women's health services, educational materials, STD screenings/counseling, and limited amounts of prepackaged medications.

Prescription Medications

The Health Center can fill prescriptions, including out-of-town prescriptions. Prescriptions can be picked up on campus at the Health Center.

Hours and Locations

The Health Center is located at 567 N. Fifth St. on campus, just north of Hulman Memorial Student Union and next to Lincoln Quad.

All undergraduate and graduate students taking one or more credit hours at Indiana State are eligible to receive services at the Health Center. Students must present a valid University ID card in order to receive services.

Students seeking services from the Health Center should bring their insurance cards to their appointments. UAP Clinic - ISU Health Center participates in many insurance plans. However, payment of plan co-payments is expected at the time of service. Any balance left unpaid by the insurance company will be sent to the person listed as guarantor.

Insurance and Fees

UAP Clinic - ISU Health Center accepts the Associated Insurance Plan, private insurances, cash, checks, or credit cards.

Students with insurance coverage need to bring their cards with them when they visit the Health Center. Plan co-payments are due at the time of service. Any balance due after insurance will be billed to the guarantor.

Students without insurance coverage are required to pay the office visit fee at the time of their visit. The fees are as follows:

  • New Patient - $20
  • Established Patient - $15
  • Preventative Visit New and Established Patient - $25

It is important to note there may be charges for radiology, laboratory, physical therapy, immunization, urgent care services, special procedures, medications, and supplies at the Health Center. In this case, a payment plan for any remaining balance can be established.

In Case of Emergency

In case of an emergency on campus, call the University Police at (812) 237-5555 to report the incident or 9-1-1. University Police will respond, contact an ambulance, and direct personnel to the site of the emergency in order to provide transportation as needed.
