Death of Spouse

No matter what the circumstances, the death of a spouse brings many unanticipated changes into your life.  These changes will result in the need to review your current benefits with the University to make sure that you have proper insurance coverage and to be sure that you don't continue to pay for benefits that may no longer be needed.  You have the option to make changes to some of the benefits provided to you within 31 days following the date of death.

Active Employees

  • Notify the Employee Benefits Office of your spouse's death.
  • If you carried Voluntary Life Insurance on your spouse, you will also need to provide the Employee Benefits Office with a copy of a certified death certificate in order for the claim to be processed.
  • If you and/or your children were covered under your spouse's health coverage, you may choose to add Health Coverage for yourself and any dependent children. To make a change or add coverage you will need to fill out the- “Medical, Dental and Prescription Drug Application” located on our "Benefits Document and Policy page".
  • Coverage will be effective on the day following the loss of coverage; premium changes, if applicable, will be taken from your pay as soon as administratively possible following receipt of the health application.
  • You may choose to open or increase the amount of your Medical or Dependent Care Flexible Spending Account.
  • You may add Vision Coverage for yourself and your dependent children, if applicable. To do so you will need to fill out "Vision Application" located on our "Benefits Document and Policy page".
  • Voluntary Life Insurance can be added/increased on yourself or your dependent children.  To do so you will need to fill out "Voluntary Life Insurance forms" located on our "Benefits Document and Policy page
  • Consider updating your beneficiaries on your life insurance, TIAA, and/or PERF accounts.
  • You may wish to adjust your federal and state tax exemptions through ISU Portal / Employee Self Service.

 

Retired Employees

  • Notify the Employee Benefits Office at 812-237-4114 of your spouse's death.
  • Contact AmWins at 1-877-517-1406 if your spouse was covered on the Medicare Supplement through AmWins
  • Contact TIAA-CREF at 1-800-842-2252 and/or PERF at 1-844-464-6777 if you were receiving monthly pension payments, which were being paid out over yours and your spouse's lifetimes.
  • Consider updating your beneficiaries on your life insurance, TIAA, and/or PERF accounts.
