Adding a newborn to your household
You may add your newborn to your Health Coverage. Coverage will be effective on the date of birth; premium changes, if applicable, will be taken from your pay as soon as administratively possible following receipt of the health application.
Because this is a Qualifying Event you may choose to add Health Coverage if you do not already have it, or you may choose to add your spouse and/or other children to your coverage within 31 days of the baby's date of birth. Coverage on the newborn and other family members will be effective on the first day of the month following the date of birth; premium changes, if applicable, will be taken from your pay as soon as administratively possible following receipt of the health application.
To add coverage you will need to fill out the- “Medical, Dental and Prescription Drug Application” located on our "Benefits Document and Policy page".
Voluntary Life Insurance can be added/increased on yourself, your spouse, or your dependent children. To do so you will need to fill out "Voluntary Life Insurance forms" located on our "Benefits Document and Policy page".
You may wish to adjust your federal and state tax exemptions through the ISU Portal / Employee Self Service badge.
