LPN-BS Track - Baccalaureate Nursing

Admission Criteria

MUST HAVE the following

• Access to a computer and be able to create, submit, and retrieve documents and documentation electronically
• Active LPN/LVN licensure
  • UNRESTRICTED in state of residence or employment
  • Equivalent in the geographical location of preceptorship or clinical practicum
• Successful completion of an approved information technology literacy course
• Continual compliance (whether in nursing courses or not) throughout LPN-BS track with
  • unrestricted LPN/LVN licensure
  • CPR certification
  • proof of negative TB status
  • professional liability insurance coverage
  • annual bloodborne pathogen training
    
• Step 1 - Unconditional admission to the University as a degree-seeking student
  • University admission requirements and regulations
  • University application and instructions
• Step 2 - Pre-Admission Testing for nursing - TEAS Exam
• Step 3 - Successful department credit by examination () of CSMs - Customized Study Modules
  • NURS 106 - Mental Health Aspects of Nursing
  • NURS 224 - Nursing Care of Adults I
  • NURS 328 - Nursing Care of the Child and Family
  • NURS 330 - Nursing Care of the Childbearing Family
• Step 4 - Complete application to the nursing program
  • Mail all pages of the application and worksheet with supporting documents:
• Step 5 - Required supporting documents to be mailed with nursing application
  1. Proof of LPN/LVN licensure
     • Legible copy of state license or license number for verification
  2. Legible copy of current / valid U.S. driver's license
  3. Proof of professional liability insurance coverage
  4. Student Health Form and Immunization form (2 forms)
     
  5. Copy of CPR Healthcare Provider Card
     • Copy front and signed back
  6. Signed RN Licensure Legal Limitations Notification Form
  7. Signed Student Outcomes Assessment Memorandum of Understanding
  8. Approved national level criminal background check
• Step 6 - May be admitted conditionally to the LPN-BS track
  1. Will be notified, by e-mail, of conditional acceptance
  2. Will be notified to register for NURS 208 and NURS 304
     • Must earn a grade of C or better in NURS 208 and NURS 304
     • May repeat course only one time
       • Ineligible for formal admission for
         • Failure of either course for 2nd time
         • Failure of both courses
• Step 7 - Formal admission into the LPN-BS Track of the baccalaureate nursing program
  1. Meet admission requirements and remain in compliance
  2. Successfully complete NURS 208
  3. Licensure as LPN/LVN is verified
     • If results are not known until after the start of the semester or if the applicant has failed the licensing examination, the applicant would be required to drop the nursing course in which he or she is enrolled and be placed into Foundational Studies courses only.
