admission application process and application form

Admission to the LPN-BS track of the baccalaureate nursing program involves:

University admission

• admission requirements and regulations
• application and instructions

Nursing program admission

• Deadline
  • June 1 for Fall semester
  • November 1 for Spring semester
• Complete the application form
  • choose SAVE when the dialog box opens (if you want to save a completed copy of the form; If a username and password are requested, click on Cancel)
  • save the document to a location from which you will be able to retrieve it if needed
  • print the application form
  • sign and date the application form
  • mail the application form including
    • copies of the required documentation indicated on the form
    • admission materials worksheet
    • by the deadline indicated below and on the form
