Educational Affiliates

Procedure to request for an Educational Affiliate:

Get a copy of the Request for an Education Affiliate ID card. It can be found at www.indstate.edu/academicaffairs/faculty_resources.htm and the form is at the bottom of the right hand column.
Fill in the following information for the person requesting educational affiliate status.
1. Full name
2. Address - this can be a business address
3. SSN
4. Work email address - this is to be entered as their personal email address.
5. Date of Birth - used in authentication to the ISU system.
6. Fill out the remainder of the form with the appropriate information for your department.
7. Send to Parking Services.
8. Parking Services will process the request and notify you of the persons 991 number and Sycamore ID
9. Have the educational affiliate log into MyISU and authenticate into the system using the 991 number from Parking Services, last four digits of their SSN and date of birth.
