Educational Affiliate Request

An Educational Affiliate is a term used to describe a person who is not employed by ISU but does have an educational affiliation. Examples are courtesy faculty and staff that need access to the electronic systems on campus. Those that are on campus may also need an ISU ID card. Educational Affiliates may teach classes, work within an office so will require the same level of access as an ISU employee.

Procedure to request for an Educational Affiliate:

  1. Get a copy of the Request for an Education Affiliate ID card. It can be found at www.indstate.edu/academicaffairs/faculty_resources.htm and the form is at the bottom of the right hand column.
  2. Fill in the following information for the person requesting educational affiliate status.
    1. Full name
    2. Address - this can be a business address
    3. SSN
    4. Work email address - this is to be entered as their personal email address.
    5. Date of Birth - used in authentication to the ISU system.
    6. Fill out the remainder of the form with the appropriate information for your department.
    7. Send to Public Safety.
    8. Public Safety will process the request and notify you of the persons 991 number and Sycamore ID
    9. Have the educational affiliate log into MyISU and authenticate into the system using the 991 number from Public Safety, last four digits of their SSN and date of birth.
