• Copy of current Registered Nurse Licensure
• Current Resume of professional work experience
• Copy of any nursing specialty certification
• Evaluation of clinical proficiency by current supervisor
• Further documentation which supports application
  • Attendance at continuing education offerings during the past five years, letter from colleagues, publications, grants, or awards, etc.
