Appeal Forms

Listed below are forms (and instructions) for appeals for undergraduate scholarships offered by Indiana State University. For ISU graduate scholarships, contact the academic unit offering the scholarship.

• Appeal for Scholarship Award:  For those students who were not awarded the scholarship but feel they met all the criteria and should have received it.
   
• Appeal for Scholarship Reinstatement:  For those students who were awarded the scholarship, subsequently lost it, and now feel they should have the scholarship reinstated.
   
• Appeal to Delay Use of Scholarship:  For those students who have been awarded a scholarship but are unable to register for full-time enrollment and wish to delay the use of their scholarship.
   
• Appeal Summer Session Use of Scholarship:  For those students who wish to make use of an awarded scholarship for a summer session.
