Required Immunizations
The following immunizations are required for
admission by the State
of Indiana and Indiana State University. Failure to submit
immunization records will result in a hold being placed on your account.
You will be unable to register for classes.

Immunization Form you need by clicking on the appropriate form.
MMR (Mumps, Measles, Rubella)
All
full-time, new and transfer students, both under-graduate and graduate,
are required to furnish a "Certificate of Immunity" for two measles, one
mumps and one rubella vaccine.
Most students will have
documentation of 2 Mumps Measles and Rubella Vaccines.(MMR) The
first MMR must be given after the students first birthday and the second
must be given more than one month after the first. The first MMR
must also be given after 1967.

The
Student Health Center offers this vaccination for a small fee if a
student is unable to find documentation of the vaccination.
The
MMR is not required for students born prior to 1957.

All
full-time, new and transfer students both under-graduate and graduate,
are required to provide documentation of a Tetanus and Diphtheria Vaccine in the last 10 years. The student
may have documentation that they received a Td, TDAP, DTap, or Adacel,

The
Student Health Center offers this vaccine for a small fee if the student
needs to have it updated.

The
Meningitis Vaccine is not required by the State of Indiana but they do
require that information be given to every student. Each student
must then sign a statement on the immunization form indicating they have
received detailed information on the risks associated with the
meningococcal disease, the availability and effectiveness of the
vaccination.

The
Meningitis Vaccine is available at the Student Health Center for a fee.

The
university also requires every domestic student to have a TB skin test
within 6 months of starting classes on campus. International students
are required to have a Quantiferon blood test. See the TB Screening page
