Immunization Requirements

     Immunization requirements for Post-secondary Level Students can be found at Indiana Code IC 21-40-5.  All students are given one semester to comply with the immunization requirements.  Failure to comply with the immunization requirements will result in an immunization hold being placed on your account preventing you from registering for the next semester.

 

Required Immunizations

Indiana Code requires that students have documentation of certain immunizations.  Indiana State University allows each student one semester to comply with all of these immunization requirements.  Failure to comply with these requirements will result in an immunization hold being placed on your record which will prevent you from registering for classes.  To see more information about the required immunizations go HERE

 

Recommended Immunizations

The Student Health Center also offers several other immunizations that are not required, but are highly recommended.  For a complete list and for information about these immunizations go HERE

 

Domestic Students

     All admitted domestic students will need to download the ISU Student Health History Form and the ISU Domestic Student Immunization Form.  These forms can be printed and taken to your family physician to be completed and either brought to the Student Health Center or you can fax it to us over our secure fax line. (812-237-7955)

     You can also send us a copy of any immunization record you may have, i.e. baby book, high school transcript, etc.  Do not send originals.  Make sure the student name, birth date and student ID# (911#) are on the record and that it is legible.

  Any required immunizations that you need can be given at the Student Health Center.  Any student failing to comply with the immunization requirements will not be allowed to register for the following semester

 

International Students

     All admitted international students will need to download the ISU Student Health History Form and the ISU International Student Immunization Form.  These forms should be completed and brought to the Student Health Center as soon as possible.  If a student needs any of the immunizations, the student can receive the immunizations at the Student Health Center.  If an international student fails to comply with the immunization requirements they can be withdrawn from the university and returned to their country of origin.

    International students must get a Quantiferon Blood test drawn at the Student Health Center or provide acceptable documentation of a TB Skin Test done in the United States.  See the TB screening page for more information 

Distance Education Students

 You are exempt from the above immunizations if you are totally enrolled in distance education and will never be on campus.  PLEASE NOTE: Check with the Department of your major to see if providing immunization records is a requirement.  Some areas within ISU require every enrolled student provide this documentation.

     If you have trouble finding immunization records that you think you have contact us and we may be able to suggest other places to check to find the records you need.

    You may be able to get needed immunizations at your county health department if you are unable to come to the student health center during our office hours.
