![]() Our services include family and educational support through dissemination of information, onsite visits, and workshops. Our services are FREE for families, school districts, and others who work with children who are deafblind from birth through 21 years of age. These services are available year-round and are provided through federal grant funds. In order for a child and his or her family to qualify for our services the child must have both a vision and hearing impairment and be included on our Indiana Deafblind Registry. Anyone can refer a student/family to the project by contacting:
Blumberg Center College of Education, Room 502 Indiana State University Terre Haute, IN 47809 800-622-3035, TTY (812) 237-3022 kgoehl@indstate.edu lpoff@indstate.edu
You also can refer a child by downloading the Adobe Acrobat Reader version of the registry form. Then just print it, fill it out and return it to Lisa Poff at the above address. If you prefer you may call and request an information packet with a census form or a fax of just the census form.
