Admission is evaluated based on the strength of your application. It should be noted that some academic units may have higher and/or additional admissions requirements. Indiana State University grants regular, conditional, and provisional admissions status. Please note that your application will not be reviewed until all required documents are received.

You will be considered for graduate Degree Seeking admission based on the following criteria:

Please note if you are applying for any of the programs listed below, you need to contact the departments prior to beginning the application process.

      • All applicants for the Doctorate in Physical Therapy (DPT) and Physician Assistant Studies (PA), programs should visit the Department of Athletic Training website and/or contact the program coordinators prior to submitting any application materials. Please note, the PA and DPT programs are cohort programs and admission is limited to the spring (PA) and summer terms (DPT), respectively. Below are the steps you need to follow to complete your Degree Application process:

  1. Apply Online here or Download Print Application.

  2. The required items below (transcripts, application fee, signature form, and affidavit) should be sent to:

    • A non-refundable check, money order, Visa Card, Master Card, U.S $35.00 payment must accompany the application form, payable to Indiana State University. Your application cannot be evaluated until the fee has been received. ISU students returning within two years from their last enrollment will be exempt from the $35.00 application fee.

    • Official transcripts for all institutions attended (undergraduate and graduate) must be sent directly from the school attended to the address above.

    • Signature form (for online applicants only)

    • Affidavit of Financial Support

  3. The following items should be sent directly to the academic program (if required) to which you are applying. Please refer to the admission requirements for individual programs published in the graduate catalog or follow up with the program director as the required documents may vary.

    • Statement of Purpose

    • Recommendation Letters

    • Any additional application materials required by your department.

  4. If your program requires any of the test(s) listed below, you must request that the scores be sent from the testing center directly to ISU.

Applicants are responsible for completing the application process in a timely fashion. Please check with your Academic Department, as they might have earlier deadlines. The College of Graduate and Professional Studies is not responsible for delays caused by mailing services. Please note that your application will not be reviewed until all required documents are received.
