The University Testing Office offers a variety of services - free of charge - to ISU faculty members, adjunct faculty, and graduate assistants.

Listed below are procedures (for credit by departmental examination and assessment of prior learning) and the necessary forms:

Test Correction and Analysis < top >

PDF Item Analysis (PDF)

PDF Job Processing Form (PDF)

The University Testing Office processes and returns tests normally within 24 hours of submission.

Marking answer sheets. Use #2 pencils only.
A Job Processing Form (at right) and a key are required FOR EACH EXAM SUBMITTED FOR PROCESSING. Please include contact information as well as information about the exam to be processed.
Answer sheets. Standard blue and green answer sheets are available in the Testing Office. Answer sheets can be picked up or sent through campus mail.
Results. Normally, tests are returned within 24 hours of submission.
Test analysis. The Testing Office provides:

  • Standard Frequencies: ID#, Name, Number of tests graded, Number of graded items, Total points possible, Maximum score, Minimum score, Median score, Percentile, Mean score, Variance, Standard Deviation, Kuder-Richardson Formula 20, and Mean Percent Score.
  • Respondent Statistics: Total score, Percent score, Z score, Deviation IQ, Number Correct, Number incorrect, and Number Missing.
  • Frequency Distribution: Percent score, Frequency, and Frequency Distribution Graph.
  • Item Statistics include individual questions: Sample Size, Number Missing, Mean, Variance, Standard Deviation, Standard Error, Median, P-value, T-value, and Point Biserial.
  • Item Analysis includes individual questions: Value, Frequency, Percent, and Point Biserial.

Printouts for each individual student are also available. For more information on this analysis, click and print our Analysis Statistics.

Key: When completing your key, use the same type of answer sheet the students are using for the exam. Fill in the answers to each question and then print the word "key" in the Name of Test box. Keys are to be submitted with each exam.
Omitting questions. If you would like to omit a question on an exam, leave that question blank on your key and complete the Job Processing Form appropriately. The question will not be scored.
Correct Answers. You cannot have more than one correct answer for a question. The scanner does not have the capability to accept more than one answer per question. Questions that have more than one answer will be omitted.
Drop/Off & Courier Services. Tests can be dropped off at the University Testing Office during office hours. Or, courier pick-up and drop-off is provided once a day at 2:30 p.m. Courier services are to departmental offices. Courier service is only available for tests.
Pick-Up. Test results must be signed for. Test results are never sent through campus mail. Please make sure that any person designated to pick up results can be trusted with your exams. The Testing Office is not responsible for misplaced exams after the exams are signed for.


 
SIR (Course Evaluations) < top >

PDF Course Evaluation Job Processing Form (PDF)
The University Testing Office distributes SIR (Student Instructional Report) forms to academic departments at the end of each semester. The academic department is responsible for distributing these forms to each faculty member.

Course evaluations are processed, returned to the department, and then distributed back to the faculty. Reprints are available from Fall of 1999.

Returning results. Forms and results can be picked up and dropped off at the University Testing Office during office hours or sent through campus mail. Our courier service is reserved for exams only; course evaluations can be sent through campus mail. Results are normally returned within 5-10 business days. The Testing Office will contact you when results are completed. Please note that we are required to send results after the grades are reported


 
Data/Survey Analysis < top >

PDF Survey Job Processing Form (PDF)

A Survey/Raw Data Job Processing Form (at right) is required FOR EACH ITEM SUBMITTED FOR PROCESSING. Please include contact information as well as information about the job to be processed.

Survey analysis. The Testing Office provides:

  • Frequencies and Univariate Statistics: Mean, Median, Mode, Standard Deviation, Range, Variance, Standard Error Mean, Skewness, and Kurtosis.

Returning results. Survey forms and results can be picked up and dropped off at the University Testing Office during office hours re sent through campus mail. Results are normally returned within 5-10 business days. The Testing Office will contact you when results are completed.

Customized answer sheets/surveys. Customization is available for ISU sponsored surveys. There is a flat fee of $50/page for development with additional printing charges. Please contact the Testing Office for more information.
