FiT for the Future
School Reservation Form
Due: March 10, 2010
Use this form to register a
group of students attending from one school.
Students attending FIT for the Future have an opportunity to attend
two, small-group workshop sessions focused on a variety of technology content areas. A description of each workshop is
provided online at www.indstate.edu/tss/FIT/workshops.htm
Please fill
out the form below with the first and last name of each
student attending. Each student MUST complete a
student reservation form by March 16 indicating their top
four choices
for workshops. Every effort will be made to register students for their
first two workshop choices, with their third and fourth as
alternates. Seniors receive top priority in scheduling.
STUDENT RESERVATIONS - Please have each student
complete the online reservation form and submit it by March
16.
After submission students should print two copies, one
for your records and one for theirs. Cost per person
(including teachers) is $10.00.
