Petition for Substitution /Waiver
If you and your Advisor determine that it is appropriate to substitute a different course for one specified in your program, print and complete this form (Form-Petiton_for_Sutstitution_or_Waiver). Once completed, take the form to your Advisor to initiate the approval process.
We encourage you to file the petition form as soon as a substitution is discussed in order to document the agreement, and so you will know that the agreement has received final approval before you register for other courses, etc.
