At this time there is no charge for the genetic counseling provided and we are happy to accept healthcare provider referrals. 

Please call 812-238-7649 and provide the patient’s demographic information and referral indication to request an appointment for genetic counseling.  Please fax any pertinent records to 812-237-4299.
