Additional Financial and Untaxed Income Form
Verification is a Federal process used to confirm students' eligibility for certain aid programs. Students who are selected for verification may be required to submit this form and other information to verify the data that they provided on the Free Application for Federal Student Aid (FAFSA). The Office of Student Financial Aid recommends only completing this form if it is listed as requirement on your MyISU Portal. For more information about verification - click here.
Dependent Student
A Dependent student is considered "Dependent" if he/she was REQUIRED to provide parental data on the FAFSA.

Independent Student
An Independent student is considered "Independent" if he/she was NOT REQUIRED to provide parental data on the FAFSA.

Note: Documents may be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and University ID number should be clearly written on every document submitted, including student and parent tax returns.
