Dependent Verification Form
Verification is a Federal process used to confirm students' eligibility for certain aid programs. Students, in dependent status, who are selected for verification must submit this form and other information to verify the data that they provided on the FAFSA. A dependent student is considered "Dependent" if he/she was REQUIRED to provide parental data on the FAFSA. For more information about verification - click here.
Note: Documents may be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and University ID number should be clearly written on every document submitted, including student and parent tax returns.
