Federal Direct Subsidized/Unsubsidized Loan Request Form

The Federal Direct Subsidized/Unsubsidized Loans will be accepted by the student through the MyISU Portal (excludes summer semester). Please follow this link for more information on this new process.

After accepting your Federal Direct Subsidized and/or Unsubsidized Loans through your MyISU Portal, any adjustments to these loans or to request an Additional Unsubsidized Loan (due to PLUS denial) will require that you complete a request form below. If you have accidentally declined your loans, you can contact the Office of Student Financial Aid to have them reinstated.

Note: Documents may be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and University ID number should be clearly written on every document submitted, including student and parent tax returns.
