Independent Verification Form
Verification is a Federal process used to confirm students' eligibility for certain aid programs. Students, in Independent status, who are selected for verification must submit this form and other information to verify the data that they provided on the Free Application of Federal Student Aid (FAFSA). An Independent student is considered "Independent" if he/she was Not Required to provide parental data on the FAFSA. For more information about verification - click here.

Note: Utilize the Secure File Upload to submit digital copies of your tax documents. Documents may also be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and ISU ID number should be clearly written on every document submitted, including student and parent tax returns.
