Untaxed Income Form
Verification is a Federal process used to confirm students' eligibility for certain aid programs. Students who are selected for verification may be required to submit this form and other information to verify the data that they provided on the FAFSA. The Office of Student Financial Aid recommends only completing this form if it is listed as a requirement on your MyISUPortal. For more information about verification - click here.
Note: Documents may be faxed, mailed, or brought in during office hours. Contact a financial aid counselor for help completing any form. The student's name and ISU ID number should be clearly written on every document submitted, including student and parent tax returns.
